Coastal Social Work can support you and your loved ones navigate the social care system, including, exploring care options, supporting evidence for an NHS Continuing Healthcare assessment.
Coastal Social Work will be able to support you with an assessment of you care and support needs, whereby we can identify what support you may need and how this can be met.
We will support you to explore what care options are avaliable, search in specific areas, specific criteria's and with your personal budgets in mind. We will arrange assessments and meetings where needed.
Coastal Social Work will continue to support you with a bi-annual or annual review, to ensure the care continues to meet your specific care and support needs.
To support professional deputies maintain regular contact and offer monitoring reports and mental capacity assessments when required. Specific social care assessment cost are as above.
To include: visit to the person, review of care support, place of residence/care provider, any relevant mental capacity assessment, professional report. The review can be tailored to the person and deputy needs.
Monthly visits will ensure that the person continues to have an advocate monitoring the care support. Each visit will include a report of the visit. The monthly monitoring service can be tailored to include annual reviews, assessment, support planning and mental capacity assessment around finances.
